Contracts & Insurance

If required, companies, consultants, and organizations shall procure and maintain at its own expense, the necessary insurance policies during the life of the written contract, agreement, or memorandum of understanding. Except for workers' compensation insurance, the policies shall not be amended or modified and the coverage amounts shall not be reduced without the District's written consent. Insurance certificates shall be provided to the District before the commencement of services.
 
The Centinela Valley Union High School District (CVUHSD) shall be named as an additionally insured (with proper endorsement) and be furnished thirty (30) days written notice prior to cancellation of any policy. The following are typical insurance requirements which may vary depending upon the nature of the service to be provided. Certificate Requirements
Certificate Type Industry-standard ACORD 25 form.

Insurance Grade Insurers with a rating at least equivalent to A.M. Best Company A:VII is required. Insurers shall be admitted in the State of California or shall be non-admitted insurers approved by the State of California. Any self-insured retentions or self-insurance must be approved by the District.
Additional Insureds Endorsement (equivalent to ISO form CG 20 10 or CG 20 26) naming “The Lawndale Elementary School District, its board, officers, agents, employees, and volunteers” as additionally insured on the appropriate policies. Such insurance shall be primary and any insurance maintained by the District, its board, officers, employees, agents, and volunteers shall not contribute. The additional insured language shall be included on the operations box AND as a separate attached endorsement for the applicable policies.
Waiver of Subrogation Must be included as an attached endorsement for the following policies, as applicable: commercial general liability, automobile, workers’ compensation, and sexual abuse or molestation. 
 
Liability Requirements
The following is a sample of liability requirements that may be applicable. The District reserves the right to adjust these requirements depending on the nature of the services to be provided. 
Commercial General Liability Coverage equivalent in scope to ISO form CG 00 01 of $1,000,000 per occurrence / $2,000,000 aggregate. Additional insured endorsement and waiver of subrogation are required.
Automobile Liability Coverage equivalent in scope to ISO form CA 00 01 of $1,000,000 per occurrence on "any auto."
Workers' Compensation Statutorily required by State of California of $1,000,000. Not required if consultant/contractor has not employees. Waiver of Subrogation is required.
Professional Liability

Required limits of $1,000,000 per occurrence /
$2,000,000 aggregate if providing professional services to the district. 
Sexual Abuse or Molestation Coverage of $3,000,000 per occurrence / $6,000,000 aggregate if working with students. An additional insured endorsement and waiver of subrogation are required. 
 
Facility Use Insurance
Facility users shall provide to the district a certificate of insurance in alignment with the above requirements with minimum limits of $1,000,000 per occurrence and $2,000,000 in the aggregate. This certificate should be accompanied by an additional insured endorsement and waiver of subrogation. The district reserves the right to require higher limits and additional lines of coverage depending on the event. 
 
If a facility user does not have coverage, the TULIP program (optional) is available and provides coverage to persons or groups who wish to use school facilities for non-school sponsored events. For more information and to apply, visit http://www.onebeaconentertainment.com/.
